## Job Description
The Community Health Program Manager will support Operations in its accountability for the Community Health Needs Assessment (CHNA) and general Essentia Health population/community health strategy. The Program Manager will serve as the primary contact assisting with the work of the Community Health Advisory Committee (CHAC).
In collaboration with the CHAC and/or designees and other community health FTEs, the Program Manager will synthesize gathered information and define a communitys health needs based on health status data and other input; identify, facilitate, and/or evaluate meaningful interventions to address health needs; and facilitate implementation of outreach, wellness, and prevention programs. The Program Manager will analyze data generated from community surveys, focus groups, and other means of community input, as well as from publically-available and internal data sources. The Program Manager will produce reports that will be used to develop and evaluate collaborative interventions in alignment with Essentia Healths population health strategy. The Program Manager will write and develop reports for public presentation and facilitate website summaries in alignment with the mission and vision of Essentia Health.
This position requires the ability to work independently and with others, as well as strong communication skills and the ability to build consensus and facilitate group meetings. Comfort with often ambiguous and evolving plans and tasks, plus the ability to coordinate sometimes conflicting goals, requirements, opinions, and timelines of multiple internal and external stakeholders is necessary.
